敕勒川,阴山下。天似穹庐,笼盖四野。

天苍苍,野茫茫。风吹草低见牛羊。

本以为这是古诗词里的场景,而我却有幸身临其境,风,是北大西洋的狂野季风;草,是一年四季都碧如翡翠,恍如童话般的寥寥草原。牛羊,黑白相间,悠闲自得。这里就是如诗如画的西欧岛国-爱尔兰。

20186月,在北京华通国康公益基金会(BHGF)的组织和医院的大力支持下,我有幸来到了爱尔兰进行为期一年的护理硕士项目学习,这一年,有汗水,有收获,有感动,现将学习情况汇报如下:

一、语言学习

到达爱尔兰后,学校先为我们安排了为期2月的语言课学习。语言课的授课形式多样,课堂氛围轻松愉快,教学方法灵活多变,极大的调动了大家学习英语的积极性。我能体会到老师们的用心和同学们学习的热情,课上大家积极发言,互相讨论,课后利用业余时间了解当地生活,珍惜每一次与当地人说话的机会。

对于两个月的英语学习,我的体会是:每天三个半小时的课堂,看似轻松的课程涉及大量的英语知识。课堂只是知识输入的一个过程,要达到好的学习效果一定要整理,记忆,输出,反馈。口语和听力是我的弱项,我坚持每天听英语广播,用英语表达自己的想法,尽量将英语应用到日常生活中,在坚持的过程中积累了经验,及时发现了问题并改正,英语整体水平也较前有了较大提升。

二、专业课学习

第一学期:

重回九月时光,第一堂专业课是领导力(Leadership),授课的是一位戴着眼镜,高高瘦瘦的女士,她看上去自信而优雅,更重要的是她给了我一个全新的自我提升的视野。在此之前的我,从未思考过什么是领导力。沃伦.班尼斯说:领导力就像美,它难以定义,但当你看到时,你就知道。杰克韦尔奇给予了领导力这样的内涵:1.真实和信任;2.不断的追求真实,不断的建立信任。在老师的指引下,我对领导力的理解为影响别人,实现愿景的能力,它是个人品行和个性的体现。这门课程中,我们对各种类型的领导力进行了深入分析,学习了领导力与组织文化、实施变革、人员策略之间的关系和相互影响。其中最让我感兴趣的是变革型领导力(Transformational Leadership),作为一种重要的领导理论它是从政治社会学家伯恩斯(Burns)的经典着作《Leadership》开始的。后来的学者Avolio将其概括为四个方面:理想化影响力、鼓舞性激励、智力激发、个性化关怀。领导力的学习指引着我对临床工作的思考,作为一名护理管理者,需要发挥模范作用提高团队凝聚力;通过创新理念迎合人们不断提高的健康需求;保持成员的学习能力提高护理核心竞争力;并通过自我领导理念和激励机制调动护理人员的积极性。既关注眼前,又关注未来;即关注环境提供的机遇,又要重视自身与环境保持良好的协调。在本学期的论文中,我分析了变革型领导力对组织文化的影响以及两者之间的关系,鉴于患者安全和同理心的文化是护理环境中最重要的文化,我重点分析了这部分内容。在讲到安全文化时,通过查阅文献和与老师的讨论,我得出得结论是将变革型和交易型领导力结合效果更佳。在论文的写作过程中,通过不断的思考和大量的阅读,我对领导力和医院文化有了更深的理解。

第二门课是护理理论(Nursing Thoery),护理理论是支撑护理学科的重要基础,这门课重在指导我们如何从临床实践中总结归纳出普遍适用的原理,如何把前辈们总结的护理理论运用到实践。其中大家熟悉的包括罗伊的适应模式(Roy’s Adaptation Mode),奥瑞姆的自护缺陷理论(Orem’s Self-Care Deficit Nursing Theory)等。最重要的是这门课使我学会了概念构建的方法。在普外科中,术后疼痛是十分普遍的现象,也给患者带来了痛苦的体验,为了提高患者术后的舒适度和满意度,我所在的胃肠外科从2015年开始试行术后疼痛管理,经过科室培训,大多数时候护士姐妹们能正确的实践术后疼痛管理方法,但有时也会出现疏忽和纰漏。查阅文献后我发现目前暂无专门针对“术后疼痛管理”(Post-operative Pain Management)概念分析的文章,于是我选择了此概念并从护理的角度进行了这门课的论文写作,从它的前因后果,属性组成、评价标准方面进行了阐述,并分析了概念所根植于的中域理论。通过概念构建,我深入了解了术后疼痛管理的内涵,对于如何从护理的角度做好疼痛管理,帮助患者实现快速康复有了更清晰的思路。

另外两门课分别是循证护理(Eevidence Based Practice)和科研设计(Research Methodology & Project Design),这两门课教我形成科研思维,用循证的方法为临床工作找依据。本科阶段的学习以专业知识为主,教我们认识什么是护理,怎样护理,是一个被动的接受知识的过程。而研究生阶段的教育以启发为主,培养我们的学习能力,教我们以已具备的知识为基础,不断拓展自己的能力,探索新的知识。老师说“所谓的科研,通常始于一个问题,终止于另一个问题”,问题会源源不断的出现,研究程度也会渐渐加深。我在这方面欠缺太多,一切都是从头学习,怎样通过外网查阅文献,搜索全文,如何利用NvivoZoteroEndnote等各种工具,如何评价文献的质量等,一学期的时间非常短暂,我珍惜时间,每天努力多学一点。在培养科研思维的过程中,我逐渐了解到隐藏在科研背后的世界观,以及不同的世界观影响下人们解决问题的不同方法。我也和导师探讨了自己的世界观,作为一名实用主义者,我专注于问题的解决,质性或量性的研究方法只要合理,都可以用在主题的论证中。

第二学期:

医学伦理(Ethical Judgementin Professional Practice)是我非常感兴趣的一门课程,国外普遍重视医学伦理,它是社会卫生事业的发展中不可或缺的一部分,学习这门课之前,我对伦理的理解仅仅是“无害于患者”,其实这只是医学伦理中很小的一部分,它涉及到我们工作的各个方面,大到医疗资源的分配,人力资源管理,医学研究的发展,小到护士工作繁忙时的小情绪,医患沟通的不顺利等,这些现象中都涉及到医学伦理的问题。医学伦理的应用并不会阻碍医学事业的发展,相反,它会促进医学事业往正确的方向前进,为人类造福。我发现在中国,护理人员的伦理行为准则比较泛泛而谈,没有过多的具体实施细节,而一些欧美国家的伦理准则详细了介绍了护士行为的各个原则,使护士在实践中有据可依,促进了护理质量的提高和护理事业的发展。

护理反思(Transforming Practice Through Reflection)是一门很实用的课程。同样的工作环境,同样的工作性质,有些人能很快进步,有些人却总是抱怨工作不顺利,命运不公平,难道真的是命运决定一切吗,我想不是的,是他们还没有明白,如何反思和发现问题以追求进步。以前我也常常思考自己工作中遇到的各种问题以求改变,反思的学习使我懂得了如何系统的分析问题以做出最佳决策,课程中我们学习了反思的方法和重要的反思理论,在我的论文写作中,我运用了“Critical Emancipatory Theory”,“Reflexivity”, “Critical Social Theory”对自己工作中曾经遇到的一个医护合作问题进行剖析,这种深度剖析帮我释怀了自己的情感,我不再陷入深深的自责中,而是真正的面对问题,思考问题,寻找解决问题的方案。

健康信息学(Healthcareinformatics)更注重护士对现代信息化发展的理解和与信息工程师的合作,信息化建设极大的促进了护理事业的发展,而护士作为护理工作的主体,对推动护理信息化建设起着不可或缺的作用,护士掌握基本的信息学知识,能为信息工程师提供有效的信息,在软件设计到使用、评价、反馈的八个步骤中,如果护理人员能有效的参与到过程中,将大大提高软件的有效性和实用性。

课程中,我们四位学员组成团队,设计了一款帮助肾病患者记录出入量的APP,这款APP方便患者实时记录自己的情况,并可按周显示出入量的动态,患者的出入量情况也可通过APP上传给医护人员,帮助他们了解患者实际情况。设计从Vision statement, Workflow diagrams, User Stories 开始,然后建立 prototype,发展,测试,邀请其他组成员使用并评价,提出问题,修改,完善,再评价等。历时两个月,完成了这款软件的运行。通过与其他学员的合作,我深深感受到团队合作的重要性,理解到信息工程师工作的艰辛,也意识到护士在信息化建设中的重要作用。

三.???? 学习体会

1.批判性思维(Critical Thinking)促进对于学习和研究的主动性。爱尔教育非常重视批判性思维的养成,要求我们在学习时开放思想,能主动去分析、探索、推理并自我校准。老师非常欢迎我们在学习时提出自己的见解并一起讨论。批判性思维的形成,有助于增强我们独立思考的能力,同时也彰显了一种人文精神。国内的教育比较依赖于一种教材并以此为“对”的标准,学习者大多通过教材的学习持有共同的观点,很少有人提出异议,即使提出异议也不容易引发群体的思考,因为大家内心依赖“对”的标准很少质疑它。而批判性思维要求在面对某个事实或目标时,我们应从多角度,多层面加以分析,减少定势思维的影响,看到事物的本质,而不是盲目相信某个观点。爱尔兰的研究生教育没有固定的教材,老师会根据课程设置给出基本的理论框架和启发引导,平时的学习需要我们自己去查阅资料和推理判断,这非常有利于批判性思维的养成。

2.反思教育促进良好学习工作习惯的养成。反思是指对学习、工作或生活中的某一现象进行剖析,发现问题,找到解决问题的更好方法。作为医务人员,每天都在匆忙中度过,上班为患者服务,下班忙着做研究,学习充电,这些都是好的,可一定要记得适当的停停自己的脚步,静下来想想自己一天都做了些什么,有什么收获,哪些地方还有所欠缺,分析原因,找到改善方案,这样学习和工作每天都会向更好的方向发展,而不再停滞不前。反思的确有神奇的力量,一次,有幸与一名大学教授交谈,我很惊讶他为什么二十多岁就可以成为大学教授,他说别人都认为他有天赋,但他认为那是他自己努力的结果。然后他拿出手机给我看,上面记录了他每天的反思笔记,大多数时候只有几句话。但他说他记的这些笔记,他一定会抽时间来学习,他认为这是他学习高效的原因。

3.严谨的科研态度形成良好的风气,促进护理科研的发展。老师们对学术。的敬畏之心深深的影响着我们,在10000字的大论文写作中,凝聚着老师的谆谆教诲和自己对学术的诚意,为了写好自己的proposal,下学期的每一天我都泡在图书馆里查找资料,准备文献和思路,然后请我的导师professorP.J.进行指导,此外每周DrawWillian博士还会为我们讲解科研设计和方法,在老师们的指导下我的思维有了很大转变,以前写论文,主要是查阅文献,找到自己需要的内容放到文章中,一年的学习使我意识到护理科研不是一味的接受别人的思想,而是批判性的学习已有的研究,通过分析、评价、概括、论证得出自己的观点。严格的论文审查制度和老师们一丝不苟的态度时刻提醒着我要有严谨的科研精神。此外,爱尔兰的护理非常重视质性研究,质性研究是基于构建主义(Constructivism)世界观的一种研究方法,它要求护士们更多的关注患者的主观感受,深入了解患者的内心,这与我们现代的护理理念是一致的-作为护士,护理的不是疾病,而是生病的人。深入了解患者的感受,更多的体现出人文关怀,提升患者良好的就医体验,有利于护理质量的提高。目前,质性研究在中国护理科研中尚属于起步阶段,护理是以病人为中心,满足其生理、心理需要的关爱行动。质性研究善于察觉患者的心理、行为和情感活动,以弥补护理中的空白,提升服务质量,有必要在临床推广应用。

四、回国工作计划

1、在科室创造良好的科研氛围,促进科研思维的养成:主要包括严谨的科研态度,批判性思维的养成,用循证的方法来解决临床问题,以及护理科研中常用的软件和学习资源的介绍。组织科室成员学习科研思路,发现和共同探讨临床问题和解决方案,申报护理相关课题并撰写论文。

2、用所学知识来指导临床实践。在校期间,我们学习了护理领导力,护理理论,护理概念、护理伦理和健康信息学等多门课程。领导力促进个人和集体的发展,护理理论注重总结归纳护理现象,将护理实践上升到理论的高度,而健康信息学重在信息化建设中,信息护士如何有效的发挥作用。护理伦理教会我们从不同角度看待工作中的各种问题,寻求最佳解决方案。如果能把这些知识有效运用到工作中,我们将会有一个清晰的视野,从而促进护理学科积极向上的发展。

3、将反思法运用于日常工作和生活中,促进工作效率和护理质量的提高。

五、结语

一年的时间转瞬即逝,我即将回到国内开展工作,非常感谢北京华通国康公益基金会和医院领导给我的这次机会,感谢同事在背后的默默付出和支持,我定会努力工作,学以致用,热爱护理事业,报效祖国,为卫生事业的美好前景贡献自己的力量。

One-year Study Experience in Ireland

In June 2018, with the support of our hospital and Beijing Huatong Guokang Foundation, I was fortunate to come to Ireland for a one-year Master of Nursing program. Now I will report my learning experience as follows:

I. Language Learning???????????

After arriving in Ireland, we took a two-month language course arranged by the school. The teaching style are varied in the class, the atmosphere are relaxed and pleasant. I can feel teachers' caring and classmates' enthusiasm for learning. In class, we speak actively and discuss with each other. After class, we use our spare time to understand the local life and cherish every opportunity to speak with the local people.???????????

For two months of English learning, my feeling is: we have 3.5 hours class time on a daily basis, seemingly relaxed courses involving a lot of English knowledge. Teaching in the class is only a process of knowledge input. In order to achieve good learning outcome, we must organize, memorize, output and feedback. Oral English and listening are my weaknesses. I insist on listening to English radio every day, expressing my ideas in English, trying to apply English to daily life as far as possible. I have accumulated experience in the process of persistence, found problems in time and corrected them, as a result, my English level has greatly improved.

2. Professional learning

The first semester

Back in September, the first class was Leadership, which was taught by a tall and slim lady with glasses. She looked confident and elegant. What's more, she gave me a new vision for self-improvement. I had never thought about leadership before. Warren Bennis said: Leadership is like beauty. It's hard to define, but when you see it, you know it. Jack Welch gave leadership such connotations: 1. Truth and trust.2.Constantly pursue truth and build trust. Under the guidance of my teacher, my understanding of leadership is the ability to influence others and realize the vision, which is the embodiment of personal character and personality. I had the honor to serve as the deputy head nurse of Gastrointestinal Surgery Department in October this year, but I felt anxious and at a loss about my new responsibilities. This course was like a beacon, giving me the direction and confidence to work hard.

?

In this course, we have made an in-depth analysis of various types of leadership (mainly including Transformational, Transactional, Charismatic, Democratic, etc.), and learned the relationship and interaction between leadership and organizational culture, implementation of change, and personnel strategy. What interests me the most is transformational leadership. As an important leadership theory, transformational leadership started from the classic book Leadership by political sociologist Burns. Later scholar Avolio summarized it in four aspects: idealized influence, inspiring motivation, intellectual stimulation and personalized care. The study of leadership has guided my thinking on clinical work. As a nursing manager, I need to play an role model to improve team cohesion: meet people's increasing health needs through innovative ideas; maintain the learning ability of members to improve the core competitiveness of nursing; and through self-leadership concept and incentive mechanism to mobilize the enthusiasm of nursing staff; focus on both the present and the future; pay attention to the opportunity that environment provides, attach importance to maintaining good coordination with the environment. In this semester's thesis, I analyzed the impact of transformational leadership on organizational culture and the relationship between them. Since safety and compassion are the most important culture in nursing environment, I focused on the two points. When it comes to safety culture, through literature review and discussion with my tutor, I have come to the conclusion that it is better to combine transformational leadership with transactional leadership. In the process of writing this paper, I have gained a deeper understanding of leadership and hospital culture through continuous thinking and extensive reading.

The second course is Nursing Theory, which is an important foundation for supporting nursing discipline. It focuses on how to summarize general applicable principles from clinical practice and how to apply nursing theories summarized by predecessors to practice. Among them, Roy’s Adaptation Mode and Orem's self-care Deficit Theory are most familiar by people. Most importantly, this course taught me how to construct concepts. In general surgery, postoperative pain is a common phenomenon, have also brought in patients with painful experience. In order to improve patients’ postoperative comfort and satisfaction, the Gastrointestinal Surgery Department began to implement postoperative pain management in 2015. After the training, nurses can correctly practice postoperative pain management methods in most of the time, but sometimes there will be minor mistakes. After reviewed the literature, I found there was no article specifically aimed at the conceptual analysis of "postoperative Pain Management" at present, so I chose this concept and analyze it from the perspective of nursing. I elaborates on its antecedent consequences, attribute and empirical referents, and analyzes which middle range theory it embedded in. Through the concept construction, I have a deep understanding of the concept, and have a clearer idea of how to manage pain from the perspective of nursing and help patients achieve rapid recovery.

The other two courses are Evidence Based Practice and Research Methodology & Project Design. These two courses taught me to develop scientific thinking and use evidence-based methods to find evidence for clinical work. Undergraduate study is based on professional knowledge, teaching us to understand what nursing is, how to care, it is a passive process of receiving knowledge. The postgraduate education is based on inspiration, cultivating our learning ability, and teaching us to expand our abilities and explore new knowledge based on our existing knowledge.

Our teacher said that "the so-called scientific research usually begins with one problem and ends with another problem." The problem will continue to emerge and the degree of research will gradually deepen. I lack too much in this area. Everything is learning from the beginning. How to search the literature through the external network, search the full text, how to use Nvivo, Zotero, Endnote and other tools, how to evaluate the quality of the literature, etc., one semester is very short. I cherish the time and try to learn more every day. In the process of cultivating scientific thinking, I gradually learned about the world view hidden behind scientific research and the different ways people solve problems under the influence of different world views. I also discussed my own worldview with my tutor. As a pragmatist, I focus on problem solving. Qualitative or quantitative research methods all can be used as long as they are reasonable. In addition, I have used evidence-based methods to explore “whether early oral feeding after gastric cancer is beneficial to patients”. It is concluded that early postoperative feeding is safe and feasible, but whether it is beneficial to patients' rehabilitation, more rigorous experiments are needed to prove it.

The second semester

Ethical Judgement in Professional Practice is a course that I am very interested in. It is an indispensable part of the development of social health. Before I took this course, my understanding of ethics is just “do no harm to patients”. In fact, this is only a small part of medical ethics. It involves all aspects of our work, from the allocation of medical resources, human resource management, the development of medical research, to nurse’s sense of burnout and mood swings, the barrier of communication between doctors and patients, the safety of patients, etc. These phenomena all involve the issue of medical ethics. The ethics of medical ethics does not hinder the development of medical undertakings. On the contrary, it will promote the medical profession to move in the right direction for the benefit of mankind. I found that in China, the ethical code of conduct of nursing staff is relatively broad and there are not too many specific implementation details, while some ethical guidelines in Europe and the United States introduce the principles of nurse behavior in detail, so that nurses can rely on it in practice. It promotes the improvement of the quality of care and the development of nursing.

Transforming Practice through Reflection is a very practical course. The same working environment, the same nature of work, some people can progress quickly, some people always complain that the work is not smooth, the fate is unfair, is it really fate that determines everything? I don't think so, they still don't understand how to reflect and discover problems to pursue progress. I used to think about the problems I encountered in my work in order to change. The learning of reflection made me understand how to systematically analyze problems to make the best decisions. In the course, we learned the method of reflection and the important theory of reflection. In my essay writing, I used the "Critical Emancipatory Theory", "Reflexivity", and “Critical Social Theory" to analyze a medical cooperation problem that I encountered in my work. This deep analysis helped me to relieve myself from distress, I no longer fall into deep self-blame, but really face the problem, think about the problem, and find a solution to the problem.

Health informatics pays more attention to nurses' understanding of modern information development and cooperation with information engineers. Information construction has greatly promoted the development of nursing career, while nurses as the main body of nursing work promote informationization of nursing construction plays an indispensable role. Nurses have basic informatics knowledge and can provide effective information for information engineers. In the eight steps of software design to use, evaluation and feedback, if the nurses can effectively participate in the process, it will greatly improve the effectiveness and practicability of the software.

In the course, our four students formed a team to design an APP to help patients with kidney disease record the amount of input, this app is convenient for patients to record their own situation in real time, and can display the dynamics of the amount of input and output by week, the patient's input and output can be uploaded to the medical staff through the APP to help them understand the actual situation of the patient. Design starts with vision statement, workflow diagrams, user stories, then builds prototypes, develops, tests, invites other group members to use and evaluate, ask questions, modify, improve, reevaluate, etc. It took two months to complete the operation of this software. Through cooperation with other students, I deeply felt the importance of teamwork, understood the hard work of information engineers, and deeply realized the role of nurses in information construction.

3. Learning experience

A. Critical Thinking promotes initiative in learning and research. Irish education attaches great importance to the development of critical thinking, requiring us to open our minds while studying, and to actively analyze, explore, reason and self-calibrate. The teacher very much welcomes us to present our own opinions and discuss them together during the study. The formation of critical thinking helps to enhance our ability to think independently, and at the same time it can be embodied in personality, highlighting a humanistic spirit. However, domestic education relies too much on one fixed teaching materials and uses this as the standard of “right”. Most learners hold the same views through the study of teaching materials, and few people raise objections. Even if objections are raised, it is not easy to cause other people thinking, because individual's mind dependence on the "right" standard and rarely questioned. Critical thinking requires that when faced with a certain fact or goal, we should analyze it from multiple angles and multiple levels, reduce the influence of stereotype, and see the essence of things, rather than blindly believe in a certain point of view. There are no fixed textbooks for graduate education in Ireland, teachers will give the basic theoretical framework and inspiration guidance according to the curriculum. Usually, we need to search for useful materials by ourselves and make reasoning judgments, this is very conducive to the cultivation of critical thinking

B. Reflective education promotes the development of good learning and work habits. Reflection is the analysis of a phenomenon in study, work or life, finding problems and finding better ways to solve problems. As a medical staff, we always spend every day in a hurry, but remember to stop your own pace for a moment, calm down and think about your day: what have been done, what has been gained, what is still lacking, find out the reasons, and the improvement plan, so that learning and work will develop on the right track every day. Reflection does have magical power. Once, I was fortunate to talk to a university professor. I was surprised that he could become a university professor in his twenties. He said that others think he has talent, but he thinks that it is his own efforts. Then he took out his mobile phone and showed it to me. It recorded his daily reflection notes, and most of the time there were only a few words. But he said that he never forgot to read these note and learned something from them, and he thinks this is the reason why he is learning efficient.

C. A rigorous research attitude forms a good atmosphere and promotes the development of nursing research. The teachers’ deep respect for academics has deeply influenced us. In order to complete my 10000-words proposal, every day of the next semester, I stayed in the library to find information, prepare literature and ideas, and then asked my instructor professor P.J. to give guidance. In addition, Dr. Drawn and Willian explained the research design and methods to us every week in the class. Under the guidance of the teachers my thinking had a big change. One year of study made me realize that nursing research is not blindly accepting other people’s thoughts, but critical learning about other studies and drawn my own opinions through analysis, evaluation, generalization, and argumentation. The strict censorship system and the meticulous attitude of the teachers remind me of a rigorous research spirit. In addition, Irish health care places great emphasis on qualitative research. Qualitative research is a research method based on the constructivism world view. It requires nurses to pay more attention to the subjective feelings of patients and to understand the inner heart of patients. This is the same as the modern concept of care - a nurse is not care for the disease, but for the person suffered the disease. In-depth understanding of the patient's feelings improve the patient's good medical experience, is conducive to the improvement of the quality of care. At present, qualitative research is still in its infancy in China's nursing research. Nursing is a patient-centered care action that meets its physiological and psychological needs. Qualitative research is good at detecting the psychological, behavioral and emotional activities of patients to make up for the gaps in nursing and improve the quality of service. It is necessary to promote it in clinical practice.

4. Work Plan

1.
